HOW TO: Enroll a Child in Child Care
Call us at 315-735-6995 and ask for a waiting list application or use the form on this page.
We will contact you schedule an time for you to come and complete paperwork. Please bring all of the required documentation to your appointment:

child's birth certificate
-
child's Social Security number
-
medical exam in the 12 months prior to the application
-
child's health insurance card
-
SNAP ID number (if you have SNAP)
-
family income information
-
for Pre-K program: proof of residency in Utica School District
Burmese, Karen, Arabic, Bosnian and Spanish speakers are available upon request to assist with registration.
